Day Admission Checklist
To ensure a smooth day admission process, we provide you with an overview of some practical details.
- For surgical day admission, please check-in at the reception of the day hospital (route 78).
- For non-surgical day admission, please check-in at the reception of the department where you will be staying (day hospital: route 78, oncology and hematology day center: route 105, or other).
- Please inform us on time via phone at +32 3 821 30 00 if you are unable to come due to circumstances or illness.
- Submit a claim to your health insurance if you have one.
- Make sure to bring your ID card, ISI+ card, Kids-ID, or alien card. For administrative and medical purposes, it is important that your information is registered accurately and completely. Therefore, we verify your data using the electronic identity card (eID), Kids-ID, ISI+ card, or foreigner's card. This card contains your official name and address details.
- Bring a list of the medications you are taking.
- If applicable, bring your referral letter, X-rays, laboratory results, vaccination card, etc.
- Wear comfortable clothing and bring something to read or relax.
- You do not need to bring toiletries, towels, or washcloths.
- Feel free to bring some food or avail yourself of the dining facilities at the UZA. No meals are provided during day admissions, but you will receive a cookie and a drink.
